10-Formyl-THF (10-Formyltetrahydrofolate) is an essential folate intermediate involved in purine synthesis, the biochemical pathway that produces the nucleotides required for DNA and RNA formation. Because purines drive cell division, immune activity, tissue repair, and overall metabolic function, this biomarker provides important insight into the health of the folate cycle and one-carbon metabolism.

The folate cycle converts dietary folates into multiple active forms, each with distinct roles. While 5-MTHF primarily supports methylation, 10-Formyl-THF provides carbon units for nucleotide production, making it especially important for rapidly dividing cells, including those in the immune system, gastrointestinal tract, and bone marrow.

Levels of 10-Formyl-THF may be influenced by dietary folate intake, gut absorption, liver function, inflammation, oxidative stress, and genetic variations affecting folate metabolism (such as MTHFD1).
