The RC-I / RC-IV Ratio evaluates the relationship between Respiratory Chain Complex I (RC-I) and Respiratory Chain Complex IV (RC-IV)—two essential mitochondrial enzyme complexes that anchor the beginning and end of the electron transport chain. RC-I initiates electron transfer, while RC-IV completes the process by transferring electrons to oxygen, enabling the final step of ATP generation.
A balanced RC-I/RC-IV ratio suggests smooth electron flow and coordinated function across the entire mitochondrial respiratory chain. When the ratio is abnormal—either too high or too low—it may indicate an imbalance between early-chain and late-chain activity, pointing toward mitochondrial stress, reduced efficiency, or functional bottlenecks.
Clinically, this ratio is useful for screening individuals with fatigue, metabolic issues, neurological symptoms, developmental concerns, or suspected mitochondrial dysfunction. Although buccal mitochondrial testing cannot diagnose mitochondrial disease, it provides valuable contextual information when interpreted alongside other markers such as RC-I/CS, RC-IV/CS, RC-II/CS, and RC-II+III/CS.
